Submit a Members Passing

This is a form to submit to put the details of a Members Passing on the Memorial Website. The Lodge Secretary must still fill out the proper paperwork to submit to Grand Lodge.
We may contact you if we need more information or to clarify details.
Please tell us about the Member who has passed away. Please tell us their Name, Lodge, Date of Death and any Funeral Details you may have. If you know the Funeral Home the service is at, please also include that for us.
